If you had a team from the 2025 - 2026 League season, we will be contacting all Team Captains to see if your team wants to come back.  You do not need to fill this out, but you can if you'd like.

If joing the scratch league, please provide your USBC # so we can get a composit average and any other additional info that may be needed
If joining the Youth League, please give us the birthdate of the youth and if they have a USBC #. There 3 levels of All Stars Youth League - All Stars 1, All Stars 2 & All Star Pee Wees
If you chose multiple leagues, please indicate which one is your 1st, 2nd, 3rd, ect choices.. Include any relevant information, team members names & team names!
